Ovaj poremećaj dovodi do nakupljanja šećera u krvi (hiperglikemije) i izlučivanja šećera u mokraći (glikozurija), a tkiva u organizmu su gladna. Zbog tih promjena jetra proizvodi energetske nadomjeske; ketone iz masnoća mobiliziranih iz perifernih zaliha. Kad proizvodnja ketona prijeđe graničnu zonu tijela, dolazi do metaboličke ketoacidoze.
Sada životinja ima vrlo jak miris po acetonu, bez apetita je, apatična (bezvoljna) i jedva se kreće, vrlo često povraća. Ovo stanje je za životinju izrazito opasno i bez odgovarajućeg liječenja vrlo brzo dovodi do uginuća.
Zbog toga u ovoj fazi bolesti ne dvojite niti trenutka, hitno životinju odvedite veterinaru.
U praksi gotovo svi psi dijabetičari trebaju terapiju inzulinom, koji se daje injekcijom dva do tri puta dnevno. Osim terapije inzulinom, psi i mačke moraju biti na strogoj dijeti koju će odrediti veterinar, a najčešće se radi o gotovim medicinskim hranama.
